About "MyHealthSocial"
"My Health Social" is a stand-alone web site (referred as "web site"). It may be also available as an application (referred as "application") on social networking sites such as but not limited to Facebook, Twitter. It may be also available as a "script" or "Application" on Healthcare Providers web sites and portals and mobile devices such as but not limited to iPhone.
After registering and obtaining user account on the web site, Healthcare Consumers can maintain list of health conditions, search and maintain list of care sources (doctors, support groups, clinical trials, web links) and create health journal, health posts. Healthcare consumers may associate their care sources, inpatient location of care providers, and health conditions to their health posts.
Healthcare consumers can link their Web site account to their Social Networks to invite friends to the Web site to form a community to post health updates to seek support. Healthcare consumer can choose to share a health note in health journal or create posting to be shared with all friends or selective friends in their community or post to public so that everybody within MyHealthSocial to see and respond with replies, recommendations and e-Cards. If a posting is made to all friends in their community, only friends in their community can see it. If a health posting is made to selective friends, health care consumer must select specific friends in their community in order for that to be seen by them. It is very important to note that if a healthcare consumer chooses to associate a health condition or a care source (a provider or a clinical trial or a support group), it will be shown whoever it has been shared with along with the health posting.
The web site (only web site) has functionality for a healthcare provider (an independent doctor, group practice, hospital, nursing home and any other provider type) to setup a service catalogue (description, services, doctors, locations, associate support groups, clinical trials) after registering and subscribing to set of services. This provides word of mouth for providers when healthcare consumers associate any part of provider's service catalogue to their health post and passed around between their friends in their community. Also same applies when healthcare consumers recommend any part of provider's service catalogue tracked as their care source to their friends in response to their friend's health updates or support needs. When healthcare consumers search for doctors or services, a local NPI database also shown so that they can still add to their list as care sources. However, those care sources will not have any rich information as a provider who has setup their catalogue in MyHealthSocial.
Terms and conditions agreement
Please read carefully. The terms and conditions defined in this document is an agreement between "you" (registered user or unregistered viewer of the site or application, also referred as healthcare consumer or healthcare provider) and Jiatro LLC (owner of "My Health Social" web site and applications) referred as "us". These terms and conditions govern use of the web site/application by you as described but not limited to the below. The "use" is defined as any click action or view action on any portion of the web site or the application.
By using this site or application in any form or shape, you conform to the terms and conditions in this agreement. This site or application shall be used As-Is and with no warranties. You agree to consent that you read and accepted these terms and conditions, privacy and advertisement policy for using the web site and application.

Access to the site and to the application
Keep your login and password secured and do not share with others. If you suspect any breach, immediately inform us.
Children under age of 15 are prohibited from registering and accessing the site as a registered user.
You will be given access to the functionality as a healthcare consumer or healthcare provider only after registration where you will supply your name, date of birth, demographics. If you are a healthcare provider, you will need to supply your identifications and will be approved for subscription only after our verification. Your access to the application is approved but not limited to agreeing to the terms and conditions, privacy and advertisement policy, e-mail verification. You are completely responsible for keeping your password and login confidential, and also responsible for all activities taken place using your credentials knowingly, unknowingly by you or anybody you willfully shared your credentials with. Sharing your password and login with others is strictly prohibited and your access to the web site and application may be temporarily or permanently suspended. You agree that any breach and unauthorized use of your credentials, your content, your personalized information, web site or application usage that you know or suspect shall be immediately notified to Jiatro LLC.
Jiatro LLC reserves right to terminate or suspend your account and access privileges with or without any prior notice for the reasons but not limited to breaches and violation of agreement, request by government agencies, lack of use by you, requested by you, technical issues. In any of such case, you agree to the suspension, termination or purge of your account and any associated content and data created by you or us or by others.
Accuracy of your information profile
Provide and keep accurate information about yourself, person or entity you are representing
You agree that any data in your profile of information provided or configured by you, automatically generated by us is verified by you for accuracy before putting to further use by you with available functionality within the web site or application. You agree that you will correct any anomalies by yourself or notify us before it is wrongly perceived by you or others to which you may chose to display. Your profile information is of the following type, but not limited to:
Entered by you during registration
- Disease conditions, care sources (doctors, services, support groups, clinical trials) you search and add to your lists
- Your health journal entries
- Your service catalogue information if you are a healthcare provider such as your company overview, doctors, services, locations, support groups, clinical trials, all of the details and links
Using "MyHealthSocial" Application on Social Networking Web Sites or third party platforms
Agree to additional terms and conditions on third party platforms. We are not responsible for limitations or restrictions of your use elsewhere.
This terms and conditions will apply to the application on all social networking web sites (such as but not limited to Facebook) or third party platforms (such as but not limited to other web sites, mobile phones etc.) wherever the application is available for use. In addition to our terms and conditions, a social networking web site's terms and condition will also apply to the use of the application on in whatever social networking site this application is available to view content or to use its functionality. Using the application by linking the accounts, adding the application on any social network where this application is available, you agree that you will abide by the terms and conditions stated here and also in that social networking web site.
Though we take measures to implement privacy, security and keeping your usage consistent on other platforms wherever the functionality and your data is available for your use, we are not responsible for any limitations or restrictions elsewhere.
Using this web site and application as a healthcare consumer

If you are using this web site or application as a registered user or viewer, playing a healthcare consumer role, you must be aware of what you can and must not use this site or application for and you agree to the following terms and conditions of usage.
You must use this site for lawful, non-commercial purpose and agree that any content you create against such will be deleted by us on our sole choice without further notice. You agree that you take full responsibility and will be truthful to any information you post about yourself. If you are creating any posting on behalf of others, you agree that you will be truthful about it and respectful to others. You agree that you do not use any search results, postings, recommendations, ratings, advises, links that you do not see valid or irrelevant to you or for anybody you represent.
You agree to use of your postings, profile information by us or our affiliates to bring relevant information to you in terms of providing matching services, products as an advise (but without any warranty) subject to the terms of Privacy and Advertisement Policy. You agree that we may disable or suspend your account without any notice or further use in case of usage against to these terms and conditions. You agree that you will report any abuse of your information or others (if you come across) to us.
Using and sharing search results
We rely upon the catalog created by our subscribed providers, NPI and service taxonomy data from government agencies and repository of other partners to provide matching results when you searched based on any criteria in our web site/application. These results may not be accurate and we do not endorse validity of these results other than displaying close matches. When you use these results as part of creating or sharing your notes, lists, postings, you agree that you will validate and responsible for ensuring it's accuracy for yours or others use. When you reconcile any internet or partner search results to items in your listing, you agree that you are responsible for accuracy of that reconciliation.
Using this web site and application as a healthcare provider
Use this site and application lawfully, truthfully to your business or other's. Report any abuse.
You agree that you DO NOT post any photographs or Personal Health Information of any patients you or your affiliates are engaged with without your business's or patient's consent. You agree that you do not abuse yours or any other business by posting any wrong information about that business such as (but not limited to) business overview, descriptions, links, services, doctors, locations, support groups and clinical trials.
You agree that you will follow your business HIPAA policies when any of the content of this application or web site (such as but not limited to e-Cards, gifts, messages) is destined to patients by their friends on social networks utilizing functionality offered by this site or application.
You agree that we may disable or suspend your account without any notice or further use in case of usage against to these terms and conditions.
Usage restrictions
You agree that you will NOT use this site and application for any of the following use and report any abuse:
Post, create, link to any information that is false, inaccurate, misleading, abusive, obscene, indecent, defamatory, libelous, threatening, illegal, hateful, harassment, soliciting, stalking, profanity, racism
- Duplicate or create any copyrighted, patented, trademarked information of others without their consent
- Disclose or share any trade secret, proprietary, business confidential information of others for commercial or non-commercial use without their consent
- Engage or influence other users to use the system in above fashion, untruthfully, to conduct any activity that is illegal and abusive, threatening to any other users
- Copying and using any content on this web site or application for commercial use without express consent from Jiatro LLC
- Use of any automated processes such as robots, scraper, spiders to access content of this site or application for any purpose and any fashion that is damaging to web site or application functionality and hosted environment
- SPAM or SPIM or bulk or any unsolicited communication
Expected or unexpected behavior of the application or web site
This web site or application may not be available or may malfunction suddenly or by planning during, before or after your use. This may be caused but not limited to emergencies, scheduled maintenance, on the fly bug fixes and changes, unforeseen threats and natural disasters. We may or may not contact you during such down time. Though we take measures, your data and information you created may be corrupted during, before or after your activity on the web site or application. You agree that you will correct yourself or bring any anomalies to our attention and work with us for accuracy and availability any portion of the web site or application.

Our liability to you in cumulative will not exceed $50 regardless of legal or equitable claim. We recommend you to cancel and close the account for any grounds of deemed liability.
Indemnification
By agreeing to these terms and conditions, you consent to indemnify and will not hold Jiatro LLC's employees, affiliates, advertisers, partners, officers, investors against any claim or action or demand or liability or settlement caused by deliberate or non-deliberate violation of these terms and conditions.
Legal governance
These terms and conditions are under legal governance of United States of America state and federal laws and disputes are litigated in state of North Carolina, but no other court or jurisdiction. Any use of this site or application outside of USA is not covered under local laws. You consent that your personal information is used as per our privacy policy and USA laws, not subject to any information and web policies of your country of residence.
Changes and transfer of terms and conditions
We reserve the right to modify these terms and conditions any time as we see needed. We may transfer these terms and conditions to our partner or parent company in case of merger or sale of the web site or application.
